Updated Forms Submission Process

We’ve made IRB submissions easier by creating fill-in forms for application documents.

Here’s What You Need to Do:

  1. Identify Your Application Type
    • Visit the Types of Applications page to determine if your application is Exempt, Expedited, or requires a Full Review.
  2. Complete Appropriate Forms
    • Fill out the application form that matches your application type.
    • Optional but recommended: Use our fill-ins for additional documents (e.g., the “consent on behalf of a child” form). You may also create your own, but we still suggest consulting our templates to ensure you cover all essential information.
  3. Attach Documents in DocuSign
  4. Set up the Signing Process
    • List the Faculty Advisor as the Primary Advisor on the DocuSign Submission page.
    • Include all researchers involved on the Submission page.
