Print your completed registration form.  Mail or hand deliver the signed registration along with your Consent & Release Form and required ISEF Forms  and $20 registration fee to: Shawnee District 14 Science Day, University Outreach Services, Shawnee State University, 940 Second Street, Portsmouth, OH  45662.  The University Outreach Office is located in the ground floor of Massie Hall, Room B52.

Registrations and all documentation MUST be received by 5:00 p.m., February 24, 2011.
