Deposit Transmittal Form

Ohio Revised Code is a set of "rules" governing state institutions and other public offices in Ohio.  One such rule stipulates that all funds received by a University department, club, or organization must be deposited at the Bursar's Office by the next business day. Likewise, the Bursar's Office is required to deposit these funds at the bank within one business day of receiving them from the department, club or organization.

Cash Receipt pr0cedures

All areas collecting money must obtain an official University receipt book from the Controller's Office on the first floor in the Administration Building.  Organizations collecting membership dues or other fees should also be providing a receipt to the individual paying.  The receipt book contains receipts in triplicate form.  The original goes to the individual making payment, the first copy goes with the deposit, and the second copy remains as a permanent record in the receipt book.

The proper form to use when making a deposit is a Deposit Transmittal Form.  To fill out this form, complete the following steps:

(1)  Enter the name of your department, club or organization.

(2)  Assign a sequential number to each Deposit Transmittal Form, if this information is helpful for you records.  This number is optional.

(3)  Enter the date that the Deposit Transmittal Form is being prepared.

(4)  Indicate the type of money being deposited (membership dues, fund-raising proceeds, etc.)

(5)  Enter the amount being deposited by type; i.e., cash denomination, coins, checks, etc.

(6)  Describe the deposit as you wish it to appear in the University computerized financial system.

(7)  If numbered receipts were used, enter the first and last numbers used for this deposit.  Also enter any receipts in this sequence that were voided.  Attach a copy of all receipts referenced in this section.

(8)  Enter the account number to be credited with this deposit.

(9)  Have the Deposit Transmittal Form signed by the authorized person. For a club or organization, this is the advisor.

(10)  Take the money and the Deposit Transmittal Form to the Bursar's Office on the second floor in the University Center between the hours of 8 a.m. and 4 p.m.  The cashier will complete the "cashier's use only" section and give you a copy of the form for your records.
