MOT Admission Packet

Below are the requirements to apply for admission to the Master of Occupational Therapy Program.  Four (4) of the requirements are forms that can be printed and mailed to:

1.  Prerequisite Summary Form for student use. Please send copy with application.
2.  Application (PDF)
The $30 application fee to be mailed with your application.
3.  Volunteer_Evalution (PDF)
of 40 hours
4.  Reflective_Analysis (PDF)
5.  References (PDF) 
Two (2) required
6.  Official transcript from baccalaureate degree
7.  Completion of prerequisite courses (Refer to Selection Criteria page)
8.  GRE scores, IF REQUIRED (Refer to Selection Criteria page)

If you are only inquiring about this degree, you may print the Inquiry Form and mail to the above address.  We will then mail you the Admission Information Packet for the Master of Occupational Therapy Program.  However, you may click on Admission Information Packet if you wish to print your own information and forms.

Entry into the program is by application and is competitive.  Review of applications begins November 1st and continues until the class for the following fall semester is full.  Approximately twenty (20) students are accepted into the program each year based on the Selection Criteria.
 

*NOTE:  Applicants are NOT required to be an Occupational Therapy Assistant, or earn an associate's degree in the Occupational Therapy Assistant Program, to apply to the MOT program.
